1. Introduction
These Terms and Conditions outline the agreement between the speech therapy service provider (“the Therapist”) and the client (“the Client”). By booking or attending a session, the Client agrees to these terms.
2. Appointments
All therapy sessions are by appointment only. The duration, frequency, and goals of therapy will be discussed and agreed upon with the Client or their representative.
3. Fees and Payment
Session fees will be communicated prior to the commencement of therapy. A payment of 50% id due prior to booking of session. Remaining Payment is due on the day of the session unless otherwise arranged. Invoices may be issued for block bookings or reports. Late payments may incur an administrative fee.
4. Cancellation Policy
A minimum of 48 hours’ notice is required to cancel or reschedule a session. This is so the session can be offered to the next person on the waitlist.
5. Reports and Documentation
Written reports, progress summaries, and other documentation can be provided upon request. Additional fees may apply depending on the nature and time required to complete these documents.
6. Confidentiality and Privacy
All personal and clinical information shared will be treated confidentially and stored securely in accordance with relevant privacy legislation. Information will only be shared with other professionals or agencies with written consent from the Client or their guardian, except where disclosure is required by law.
7. Communication
The Therapist may contact clients via phone, email, or text to confirm appointments, provide updates, or share relevant resources. The Client is responsible for ensuring their contact details remain current.
8. Therapy Attendance and Participation
Active participation by clients and families is essential to achieve therapy goals. Home practice and collaboration with carers, teachers, or other professionals are encouraged to support progress.
9. Termination of Services
Either party may choose to discontinue therapy with appropriate notice. The Therapist reserves the right to terminate services in cases of non-payment, repeated cancellations, or where therapy is no longer appropriate or beneficial.
10. Liability
While every effort is made to provide high-quality, evidence-based care, the Therapist is not liable for outcomes dependent on factors beyond their control, including consistency of attendance or engagement with home programs.
